Q. The program is making chapters and books that don't exist!

A. The program is looking for anything that it considers to be a chapter name or number or book name.
If it encounters a number or roman numberal on the start of a line with less than 6 characters it thinks it a chapter number.
If it encounters the word Chapter, or CHAPTER it considers it to be a chapter name.
If it encounters the word Book or BOOK at the start of a line it considers it to be a book name.

Solution
Move the number or word to the end of the previous line or, move the word on the end of the previous line to before the number or word.
